Leg Length Discrepancy and Back Pain: How Chiropractic Care Can Help

September 29, 2026

Does one pant leg always seem to drag, or does your back ache after a long day on your feet? You could be dealing with a leg length discrepancy (LLD), a surprisingly common difference in the length of your legs. Small differences are normal, but when one leg is noticeably shorter, the pelvis can tilt toward that side and the spine curves to compensate. Gradually, that unbalanced foundation can stress the joints, discs, and muscles of the low back.

WHY ACCURATE LEG LENGTH MEASUREMENT MATTERS

Research shows that measuring a leg length difference takes care and consistency. Tipton, Sutherland, and Schwarzkopf (1) reported that measurements from a standard pelvic X-ray differed significantly from true leg length, and encouraged assessing any existing discrepancy before hip replacement surgery. Kadhim, Kelly, and Sabharwal (2) found that three full-length standing X-ray methods produced comparable results, but suggested using one technique consistently, especially when the legs bow or angle. Just as important is where the difference begins. Tolk, Eastwood, and Hashemi-Nejad (3) linked leg length differences in Perthes' disease to the top portion of the thigh bone near the hip. Birch, Makarov, and Jo (4) found that when one part of a child's leg is shortened, the unaffected part of that same leg generally doesn't grow longer to compensate. You can trust your chiropractor to figure it all out for you!

HOW CHIROPRACTIC CARE CAN HELP WITH UNEVEN LEG LENGTH

This is where your chiropractor can make a real difference. Chiropractic's greatest contribution is identifying the discrepancy and understanding how it changes spinal mechanics in ways that can cause pain. Your assessment can include checking pelvic level, posture, and gait, along with X-rays when needed. Treatment can then include precise spinal adjustments, flexion-distraction to reduce pressure on aggravated joints and discs, individualized exercises, and heel lifts as needed. If a discrepancy calls for orthopedic evaluation, your chiropractor can certainly coordinate that referral.
